SELF-PROVED WILL

 

     Each of the undersigned attesting witnesses to the Codicil to the Will of Blaine Benson Bobbson1 dated August 19, 19962, of which this Affidavit is a part, first having been sworn on oath states that said testator stated to us that the above and foregoing instrument was Blaine Benson Bobbson's3 Codicil to Last Will and requested each of us to witness Blaine Benson Bobbson's4 signing of said Codicil to Will, that each of us was present and saw the said testator sign said Codicil to Will in the presence of each of us, that said Codicil to Will was attested by each of us in the presence of the said testator and each of other and that each of us believed said testator to be of sound mind and memory at the time of signing said Codicil to Will.

  2. The date on which the testator signed the Codicil.

  3. The name of the testator.

  4. Again, the name of the testator.

  5. The signature of the first witness.

  6. The address of the first witness.

  7. The signature of the second witness.

  8. The address of the second witness.

  9. The state where the document is signed.

  10. The county where the document is signed.

  11. The testator who signed the Codicil.

  12. The name of the first witness.

  13. The name of the second witness.

  14. The date on which the Codicil was signed.

  15. The signature of the notary or other official.

  16. The identity of the capacity of the individual signing.  In this case, a Notary Public.
